 We assist an individual who is age 18 or older to participate more meaningfully in home and community life. It is provided to individuals living in private homes for the purposes of providing supervision or assistance that is designed to ensure the individual’s health, safety and welfare or to perform activities of daily living for the individual.

This is a direct service (face-to-face) provided in home and community settings to assist participants in acquiring, maintaining, and improving self-help, domestic, socialization, and adaptive skills necessary to reside successfully in home and community-based settings. Services consist of support in the general areas of self-care, communication, fine and gross motor skills, mobility, personal adjustment, relationship development, socialization, and use of community resources.

Residential Group Home

Our group homes offers 24 hour supervision in a home environment for individuals who would like to learn and develop skills necessary to achieve a greater level of independence. This type of residential program offers supports customized to meet individual needs, desires and outcomes.   Services include: • 24 hour staff supervision • Coordination of educational, vocational, medical, mental health, social and financial services • Advocacy • Transportation and recreational activities

Residential Group Home

These are services that are provided to supervise and support individuals living in private homes on a short-term basis for planned or emergency situations, giving the person(s) normally providing care a period of relief that may be scheduled or due to an emergency.

Supported living

 Supported Living is a residential option available to those individuals requiring less than 30 hours of staff support per week. These individuals reside in apartments or homes of their own and staff provides support to them as needed. Supports include: assistance to medical appointments, money management skills, cooking skills, and community integration.
